This nomination form is for both the Susan E. Bennetto History Student Recognition Awards and the History Student of the Year Award. Please review the nomination form and have your supporting files ready for upload prior to filling out the form. It must be completed in full and cannot be partially saved, but you will have the opportunity to preview your completed form prior to submitting.

The Susan E. Bennetto History Student Recognition Awards (for grades 11 & 12 students)
Award is for students in grades 11 & 12. Students are to fill out the form themselves, and provide contact information for those who support their nomination (teachers, department heads, school administrators, museum/heritage sector workers.)

The History Student of the Year Award (selected from among Bennetto recipients)
Winner and two runners-up are selected from among Bennetto Award nominees.
