The following forms are available for ordering publicly funded vaccines:
- Routine Vaccine Order Form for all vaccine orders except for Hepatitis A, Hepatitis B, HPV, Meningococcal A, C, Y, W135, and Meningococcal B.
- School Vaccines Order Form for Hepatitis B, HPV, and Meningococcal A, C, Y, W135.
- High Risk Vaccines Order Form for Hepatitis A, Hepatitis B, Meningococcal A, C, Y, W135, Meningococcal B, and HPV.
- Tuberculin Special Order Form for Publicly Funded Tubersol
- Fall Respiratory Vaccines Order Forms
- Non-Reusable Vaccine Wastage Return Form to be used when returning vaccine to Public Health
Fax the vaccine order form(s) to 905-546-3472 along with 4 weeks of twice daily logged current, minimum and maximum temperatures. The vaccine will not be sent if we do not receive log sheets. Ensure that your facility’s name is clearly identified on the temperature log sheets.
Cold chain incident and wastage reporting
To report vaccine refrigerator temperatures outside the 2°C and 8°C range, call 905-546-2424 ext. 4777
Adverse events reporting
Health professionals are required by law to report adverse events following immunization that meet certain criteria. If you are not certain whether an event meets reporting criteria call 905-546-2424 ext. 7556 to speak with a public health nurse.

Publicly-funded immunization schedules for Ontario
These schedules specify who is eligible for vaccines free of charge. There is a routine childhood immunization schedule, catch-up schedules, schedules for adolescents and adults and high‑risk criteria.
