How to make a referral
Refer your clients to Alcohol, Drugs and Gambling Services by faxing the referral form to 905-546-3608.

You can refer clients to the following services:
Information & Intake
Information and intake is for family, friends, professionals and people concerned with their own or others alcohol or drug use or gambling.
- Over the phone or at a drop-in
- Drop-in intake is available on Mondays from 2 to 4 pm and Thursdays from 9:30 to 11:30 am at 21 Hunter Street East on the 3rd floor
Individual counselling
Assessment & referral
- Free, confidential, individualized assessment where we help people identify their needs and create a plan for treatment
Case management
- For people with complex issues who need support to connect with other services or to maintain change
Community treatment
- Individual and group formats
- Cognitive behavioural therapy, structured relapse prevention and dialectical behaviour therapy are offered within a philosophy of harm reduction and self-determination
Group Programs
Orientation group
- One session to provide information, start a plan and complete assessment tools
- Treatment options are reviewed and help with next steps is provided
Skills for Change
- 12 week education group
- Helps people increase or maintain their level of commitment to change and to develop coping skills
- This is an open group after completing the intake
Relapse prevention
- Facilitated group for people who had previous treatment for their substance abuse and believe they are at risk to relapse
- The focus of the group is to review, practice and implement skills that reinforce abstinence
Older, Wiser Lifestyles
- To help adults age 55+ concerned about their alcohol use, prescription drug use or gambling to make healthy lifestyle choices
- Seven educational group sessions are offered and ongoing support groups
Skills to Enhance Personal Success
- Also known as STEPS
- Dialectical behaviour therapy group for people struggling with changing their substance use or gambling
Healthy Lifestyles Change Group
- Offers support for those with a severe and persistent mental illness such as schizophrenia who have concerns with substance use or gambling
- This is co-facilitated with the Wellington Psychiatric Outreach Program
Crossing Paths
- Cognitive behavioural therapy group
- Offers support to people who have mood and/or anxiety issues and substance abuse
Bets Off
- Facilitated weekly support group
- Helps people with gambling concerns maintain their goal of abstinence through group support and relapse prevention strategies
Survivors and Substance Use
- 10 week group
- For women who are survivors of sexual violence and have concerns with substance use
- Co-facilitated with SACHA
Other programs
DrinkWise
- Education program for people with mild to moderate alcohol problems
- Individual based, but referrals can be made to a DrinkWise group
Significant Others
- Individual counseling for people impacted by someone else’s gambling
Methadone Case Management
- Assessment and ongoing support to people who are on methadone maintenance therapy
- Services are linked closely to other community resources
Choices and Changes
- Offered at Hamilton Children’s Aid Society
- For people involved with child welfare to assess, make recommendations for treatment and case manage issues related to substance abuse, parenting and children at risk
- A two session, two hour group is offered for those in pre-contemplation and contemplation stages, exploring the impact of substance use on their lives
Beginning Today
- For individuals receiving Ontario Works benefits
- Addiction services include individual and group counseling for addictions, referral to residential treatment and consultation with OW staff
- Groups include: Possibilities Now, an orientation group; Action Becomes Change, a 12 session group; Re’Preve, a closed relapse prevention group; art therapy; music therapy; and a collective kitchen
- Individuals need to speak with their OW case manager for a referral
Cravings Hotline
- Call 905-546-3606 and press 6 to hear messages providing information and strategies to help with cravings for alcohol, drugs or gambling
- Open 24/7
Offsite outreach services by Public Health
- For individuals who are homeless or at risk to homelessness
- At The Street Health Centre at the Wesley Centre, 195 Ferguson Avenue North on Friday from 1 to 4 pm and Hamilton Urban Core Community Health Centre, 71 Rebecca Street on Tuesday from 5 to 8 pm
- No appointment is needed
Back on Track
- An assessment, education and treatment program for people convicted of impaired driving
- Alcohol, Drugs and Gambling Services is the local provider of this mandated service
- Call 1-888-814-5831 to register

HOW TO MAKE A REFERRAL
Refer your clients to the Public Health Quit Smoking Clinc by faxing the referral form to 905-546-4194 or have client's call the Tobacco Hotline to book an appointment at 905-540-5566 ext 1.
