Family Doctor: connect with your family doctor to ensure your child's vaccines are up to date.
Walk-in Clinic: if you do not have a family doctor but you have a health card or

How to Report Vaccines Online

Parents are responsible for notifying Public Health each time their daycare or school-aged child receives a vaccine. In the event of an outbreak, Public Health needs to have the vaccine information for all daycare and school-aged children in order to protect the community from vaccine preventable diseases.

Avoid the risk of school suspension as certain vaccines are required by law to attend school or daycare.

Please remember to report your most up-to-date vaccination records each time you receive an immunization. We highly recommend that you review the publicly funded immunization schedule

Please consult with your health care provider (HCP) to determine what vaccines your child needs. If you do not have access to a HCP, you may consult with Health Connect Ontario for any consultation needs, or Hamilton Public Health Vaccine Program Nurse Infoline at 905-546-2424 ext. 7556. Please note that the Vaccine Program is only available between 8:30 am to 4:00 pm, Monday to Friday (excluding holidays), and Health Connect Ontario is available 24/7.

The fastest and most convenient way to submit and review updated immunization records is to access the Immunization Connect Ontario or ICON portal at hph.icon.ehealthontario.ca.

Please note that the site is maintained by the Ontario Ministry of Health and Long-Term Care, and not the local Public Health Unit. This site should NOT be used to record your COVID-19 vaccination. If you upload your COVID-19 vaccine record on this site, it will be rejected.

If you don’t have access to the internet or an internet enabled device, or if your provider is reporting on your behalf, your child’s immunization record may be mailed to:

Access ICON by going to hph.icon.ehealthontario.ca
Enter your Ontario Health Card Number or Ontario Immunization ID (OIID), and set or enter your PIN to securely log in.
Enter the vaccine history.
Attach a photo or a scanned copy of your physical vaccine record(s) and submit.

Please note: To ensure our vaccination information on file is accurate, a copy of the vaccination record or a paper/card from your health care provider must be attached when submitting the vaccination information through ICON. If this information is missing, we may be unable to process the vaccination information.

Please ensure you have your Ontario HCN or OIID ready along with other identifiers such as first and last name and DOB, etc. If you can access the database and no record is found, it is likely due to the record having never been reported to Public Health. If you are experiencing issues with accessing ICON, please contact us at 905-540-5250.
