Healthy Smiles Ontario
Healthy Smiles Ontario is a government-funded dental program that provides free preventive, routine, and emergency dental services for children and youth 17 years old and under from low-income households.
Mandatory and discretionary dental services are available to those receiving Ontario Works, Ontario Disability Support Program (ODSP), Assistance for Children with Severe Disabilities (ACSD), low income and their families.
Ontario Disability Support Program (ODSP) Adult and Spouse (18 years and older)

Who can apply?
- Children 17 and under are automatically enrolled and will receive dental benefits from Healthy Smiles Ontario when they or their family receive ODSP and ACSD.
- Children 17 and under will receive a Healthy Smiles Ontario dental card and information about the program in the mail.

For ODSP Dependent Adults 18 year of age and older:
- ODSP dependent adults are not covered under the “ODSP dental card”
- ODSP dependent adults are to call Special Supports at 905-546-2590.
- Please refer to the Low Income and ODSP Dependent Adult section below.
Ontario Works Children's Dental
Who can apply?
Children (0-17) are automatically enrolled and will receive dental benefits from Healthy Smiles Ontario when they or their family receive Ontario Works.
- Children 17 and under will receive a Healthy Smiles Ontario dental card and information about the program in the mail.

Ontario Works Adult Dental
Who can apply?
Ontario Works Adults (age 18 and over):
- Participant to provide their “Ontario Works Dental Benefit Eligibility Card” for the month of treatment to their dentist of choice
- A Dental Benefit Eligibility Card for the month of treatment must be provided to the dentist, this is provided monthly with the Ontario Works cheque/direct banking deposit statement
- Dental Card replacement: If you require a dental card replacement please call the Ontario Works Program 905-546-4800 and press #3. A dental card can be faxed to the dentist of your choice and/or mailed to your home address.
- City of Hamilton sets the Schedule for the Discretionary Adult Emergency Dental Treatment Plan Services and Fees for this program.
Ontario Disability Support Program Dependent Adults age 18 and over (other than applicant or spouse) and Low Income Adults (18 to 64 years) Dental Program
- ODSP dependent adults are to phone Special Supports at 905-546-2590
- Low Income Adults (18 to 64 years) must complete an application to determine eligibility
- Participant can choose the dentist of their choice
- Participant to provide their dentist with the Special Supports Dental - Approval letter

Special Supports Program

- Dentist can treat the maximum 4 symptomatic teeth per emergency or treatment visit (any combination of fillings, root canals on the anterior front teeth (1.3-2.3, 3.3-4.3) and/or extractions).
- Limit of 4 emergency or treatment visits per 12-month calendar period effective Jan-Dec. Please see the remainder of the fee guide limitations for each procedure.
- Pre-authorizations are no longer being accepted, EXCEPT in cases where full mouth clearance is required prior to the patient obtaining dentures. For all other dental work, please follow the fee guide. For further information on how to submit a pre-authorization for full mouth clearance, please see the fee guide. Please note: a copy of the pre-approval form is now required with each claim submitted for payment.
- Pre-approvals will be honoured for 6 months from the date of approval. Any submission after this date will be processed in accordance with the fee guide limitation of 4 emergency or treatment visits per 12-month calendar period Jan-Dec.
The Discretionary Adult Emergency Treatment Plan has some limited and restrictions.
Items that may be covered (within limits and criteria):
- examinations, x-ray and treatment limitations
- fillings- both silver and white
- restorations
- root canal-front teeth only
- extractions and surgery
- nitrous oxide with limits
Items NOT covered:
- Cleaning and scaling
- Root canals on back teeth
- Crowns and bridges
- Primary (baby) teeth
- Orthodontics of any kind
- Night guards
- Sedation

Low Income
- You must complete a Low Income application to determine financial eligibility and include the denture predetermination. You can mail these to the Special Supports Program.
Ontario Works and/or Ontario Disability Support Program Recipients
- At the participant’s request the Dental/ Denturist office can send the predetermination to the Special Supports Program either by fax or mail.
- When the predetermination is approved, the Special Supports Program will mail the applicant an approval letter to take to the dentist.
