Financial Support for Child Care

1. Proof of your status in Canada for all family members (only required at initial appointment or if status changes)

• Canadian birth certificate; or
• Valid Canadian passport; or
• Permanent resident card; or
• Work Permit; or
• Record of Landing; or
• Refugee papers’ or
• Letter from Citizenship and Immigration (CIC) verifying permanent resident status or application for status

2. Proof of your income (one of the following for all adults in the family)

• Canada Child Benefit Notice
• Current Federal Notice of Assessment
• Ontario Child Benefit Notice
• Families receiving Ontario Works/ODSP support do not need to verify income
• Review the chart to confirm the tax year to submit: 

3. Proof of the reason for needing child care (one of the following for all adults in the family)

• One month of current paystubs or a letter from employer verifying hours working each week
• Verification of school enrolment i.e. OSAP Assessment, class schedule, including confirmation of start and end dates
• For self-employed applicants one of the following: Master Business License; OR Business Name Registration; OR Official document stating HST number; OR Certificate of Incorporation and your most recent Federal Notice of Assessment. All self-employed applicants must also complete a Self-Employment Questionnaire(PDF, 257.3 KB). One questionnaire is to be completed for each self-employed applicant.  If both applicants are working for the same business only one questionnaire is required.

4. Proof of your current address

• Recent utility bill
• Lease or mortgage papers
• Letter received from recognized institution such as bank, credit card or school

5. Proof of custody arrangements (if applicable and only required at initial appointment or if/when custody changes)

• Legal documentation, divorce papers, separation agreement, mediation report or other written agreement verifying custody arrangements

Due to high sensitivity of information and for your protection, we do not accept any documentation by e-mail in the form of attachments, scans or photos. Do not submit original documents. Copies of the following supporting documents must be faxed, mailed or hand delivered to the following address.

Everyone has the right to appeal an eligibility decision. Appeals must be submitted in writing with any new information relevant to your situation. All appeal decisions will be sent by mail to your home address. Subsidy Eligibility Appeal Form(PDF, 138.44 KB).

1. Visit My Child Care Account via https://www.missioninc.com/cso/hamilton/en-ca/services
2. Select “sign in” in the top right corner of screen.
   
3. Select the “Create an Account” command.
   
4. Complete the required information (first name, last name, date of birth, email and password) and select  the “create account” button.
   
5. Shortly after, you will receive a confirmation email from My Child Care Account. Sign into your email, find the message from My Child Care Account and click the link within the email to confirm your account.
   
6. After confirming your account, you will automatically be redirected to the sign in page of My Child Care Account.
7. Call or email your subsidy eligibility worker to initiate your account.
8. Once initiated by your subsidy eligibility worker, you will be able to send messages, upload documents, and update your address on My Child Care Account safely and securely.

My Child Care Account allows you to quickly and conveniently update your address and phone number.

You can update your information in your My Child Care Account by following the below steps.

1. Sign into Child Care Account via www.missioninc.com/cso/hamilton/en-ca/services
   
2. Select the person icon and select “profile” from the drop-down menu
   
3. Select “update address” or “update phone” to make required changes
   
4. Complete the required changes and select the “submit” button
   
5. You will receive a confirmation message when your address / phone number change is successful

My Child Care Account provides a convenient, safe, and secure way to send messages and upload documents to your subsidy eligibility worker. You may be required to provide documents for:

• Determining your initial and ongoing eligibility for Child Care Subsidy 
• To provide verification of changes such as new employment or address changes

Before you upload any documents:

• Ensure documents are clear and legible (i.e. no blurred photos) and all required information and pages are included.
• Make sure the file(s) you want to upload do no exceed 4 MB.
• Allowed file types are: PDF (.pdf), JPG (.jfi, .jfif, .jif, .jpe, .jpeg, .jpg)

You can upload to your My Child Care Account by following the below steps.

1. Visit My Child Care Account via https://www.missioninc.com/cso/hamilton/en-ca/services
2. Select “sign in” in the top right corner of the screen.
   
3. Select “Messages”
   
4. Select the “New Message” button.
   
5. Complete the “title” and “body” sections.
6. Select the “Add Message” button.
   
7. From this screen you have the option to upload a document by selecting the “Select Files” button.
   
8. Choose the file you would like to upload and select the “open” button.
   
9. You will now receive a message to show your file was successfully uploaded.
