As part of the Youth Steering Committee, you will have the option to submit photos for the @HamOntYouth Instagram Channel. We also like to take photos of Youth Steering Committee meetings. This is the City of Hamilton's Photograph and Information Permission form. By completing this, you are agreeing to letting the City of Hamilton utilize these photos.

The undersigned authorizes that City of Hamilton to permit photographers/videographers employed or designated by the City of Hamilton to take or obtain photographs and/or file/video footage of and to obtain quotations and information from the undersigned. Such photos, film/video footage and recorded comments may be used for publications and broadcasts which may include but not be limited to the following: newspapers, radio, television, staff newsletters, news releases, pamphlets, brochures, flyers and promotional publications and the undersigned shall be entitled to no compensation as a results of such use by the City of Hamilton.

The City of Hamilton collects information under authority of Section 227 of the Municipal Act, 2001. Any personal information collected for the HamOntYouth Steering Committee will be used for registering committee members. By providing your contact information including email address, you are consenting to receiving emails from the City of Hamilton and/or their agents/contractors for the HamOntYouth Steering Committee.
